Piercing Consent, Placement and Jewellery Records: PDF Pack

An eight-page fillable piercing pack with body, face and ear maps, consultation, consent, jewellery records, follow-up and separate photo permission.

Complete piercing consent, body-map and ear-map pages from the eight-page pack.

A piercing record needs to identify the actual site, client side, agreed procedure and jewellery. An ear diagram alone cannot do that for a facial or body piercing. This eight-page pack gives professionals a common consultation and consent record with optional body, face and ear maps, plus procedure and follow-up pages.

In Glamorph, client documents can sit with the client or dated session, alongside notes and photos. Attach a completed PDF or use the separate document workflow to prepare and sign a document in person or remotely. Neither route requires publishing an online booking page. Attaching this PDF does not convert its fields into structured app data.

1. ConsentRecord the procedure, location, jewellery, discussion and signatures
2. ConsultationRecord relevant disclosures, anatomy and suitability discussion, questions and the agreed decision
3. Body mapMark a general body area and document the exact site and side

7. Aftercare and follow-upRecord advice, review findings, jewellery changes or downsizing and the next step
8. Photo permissionRecord separate choices for identified images and named uses

Use whichever map is relevant. The client does not need to complete three location sheets for one piercing. Existing consultation records can be referenced instead of duplicated.

Body and facial piercings need an exact written site

The maps are blank surfaces for discussion, not recommended piercing locations or anatomy assessments. Number each mark and record its exact site and client side. General outlines do not show every oral or intimate location. Attach an appropriate site-specific drawing or reference when needed. A broad body circle should never be the only placement record.

The Association of Professional Piercers’ client-rights guidance emphasizes discussion of individual anatomy, jewellery, risks and aftercare, as well as the client’s ability to stop. The pack leaves those discussions to the professional and client rather than supplying default sites, dimensions or suitability answers.

Record jewellery changes as dated work

For a new piercing, change, stretch or later downsize, identify what was actually done. The procedure table has room for the site, material, supplier, gauge, dimensions and batch or traceability reference. Use dimensions appropriate to that item and record the units. Link the relevant sterile-item record instead of entering a generic assurance.

At a review, use a fresh follow-up sheet. Keep observations and client-reported concerns distinct from the action taken. Reference the current written aftercare supplied. This workbook does not prescribe a care regimen. The APP aftercare resource is a professional reference to consult alongside the advice appropriate to the actual piercing.

Signatures and photo choices

This pack is for adult clients. It does not supply a minor/guardian consent process or certify compliance with every local rule. Adapt the consultation and consent wording to your service, licensing and insurance requirements before using it.

Photo permission is a separate page with four unchecked choices. Specify the actual images, channels, restrictions, use period and withdrawal contact. Agreement to the piercing does not authorize portfolio or social-media publication.

Fill the text fields in a compatible PDF reader and mark maps with a pen or annotation tool. Sign by hand or with a reader signing tool. These are not certificate-signature fields. Save a new completed copy and reopen it to check the entries and choices. The PDF does not transmit client information to this website.

In Glamorph for piercing professionals, create the client and dated session, then attach the completed document. Add the procedure notes and relevant photos to the same client history, so the next review starts with the agreed site and jewellery record.
